The Oklahoma Hospice & Palliative Care Association was successful in getting legislation passed during the 2011 legislative session to authorize a hospice and palliative license plate.

  Now we need your help!

 Before the license plate can be ordered or manufactured, the Oklahoma Tax Commission must have 100 prepaid applications on file by May 1, 2012. 

 The hospice specialty license plate is a beautiful and thoughtful way to show others the importance of having quality end-of-life care.  The plate can be purchased by:

 For every plate sold, $20 will go to support the mission of the Oklahoma Hospice & Palliative Care Association.

The Design on the front of this card is a sample only.  Finalization of design is in process.  Once a plate is approved for issuance, please allow 6 to 8 months for processing.

If an insufficient number of applications are received by May 1, 2012, all application fees will be refunded.
